If you'd like to share your story, you can:

Type your story into the text panel in the form below and attaching your photo* or

Create your own document (eg. Microsoft Word, Apple Pages, PDF etc) and send it to us by attaching the document and your photo* to the form below or

Send it to us at hello@alwaysonmymind.co and attach your photo*​​

Including your photo is optional.
If you do send it to us, we'll publish it in B/W so it'll be consistent with all the stories.
Please note that by sending us your story (and photo*) you are granting us permission to publish it on our website.
You can contact us at any time and request any changes or updates to the content of your story and/or your photo. Updated stories will re-appear at the top of the list with an indication that it's been updated. You can ask us to delete your story at any time.
